One thing to note about this, A4 and A5 aren't used in North America (I don't have a ruler handy to see what they are)

According to my Printer Page Setup,
  • A5 would be 14,82x20.99cm
    A4 would be 20.99x29.70cm
    US letter would be 21.59x27.94cm
I'm assuming that's the standard size in the US… Have to think about that. Generous margins? Gantry get that ruler out!

Here in North America, we use US standard sizes:
Letter = 8.5" x 11"
Legal = 8.5" x 14"
Tabloid = 11" x 17"

Our photocopiers and printers often support the ISO standard sizes, but good luck finding paper.

Right… so, if go by A4 width (20.99cm) and US letter height (27.94cm), then the layout will fit both…

I just discovered something that might be of help, although I haven't tested it, if someone does please post your results.

In Adobe reader, there is a booklet printing function in the Print menu.  In Adobe Reader, go to File, Print, look in the Page Handling box.  In Page Scaling, choose "Booklet Printing".  That should create pages that are scaled down by half, rotated 90 degrees, on one page.  Thus you should have your 8.5" x 5.5" booklet size without having to make any changes to the original.

I just discovered a problem with the above anyhow... if your browser is configured to open the pdf inside your browser, "Booklet Printing" isn't available.  If your browser is configured to, or you decide to open the pdf in an external Adobe Reader window, only then is "Booklet Printing" available.  This would be too confusing to rely on, so a resize of the document makes more sense in the long-term.
