It's both! You gotta love what Hans im Glück has done with the game. Expanding to unfortunate new ends, creating expansions in the guise of spin-offs, releasing three different versions of the same expansion. Fun times.
can we add the wheel pf fortune with the before sets of carcassonne?
if yes, do we add ALL 72 tiles(for a 3 days 7 hours 52 minutes game) of just the starting tils AND the 19 tiles with the icon??

You can do it however you want. Personally, I plan to remove the 19 Wheel tiles and keep them separate from the rest as an expansion. Wheel of Fortune is completely compatible with all the other expansions made for Carcassonne and there are no rules contradictions I know of. Add in the tiles if you want an extra-long game, leave them out if you don't! It's your choice.
