What happens when someone noticed a wrong played tile, some turns before? Do we keep it? Do we remove it? What if it scored a feature?

nothing much, we keep it in play as played... if nobody see it before a few turns, everybody & nobody's guilty then!.

A match can never be ended on a defensive penalty!!!

Well, if it was just a few tiles back and we remember what happened since, then we just "take back" the tile until the mistake tile and the player gets to play it legally. Then the next player(s) uses the same tile they have drawn and play as usual. If it was too long ago, then we just decide to just keep it in play as Deatheux has said.
When we play though, we usually check every tile placement when it is made so that we don't make the mistake of placing a tile in an illegal position.
Edit: I wonder what's the official position on this? Perhaps we should ask HiG? Joff, could you add this to your list?

i doubt that HiG can take position on this, nobody is error proof! even multiplayers games can have a failure ...

You should realistically never have an erroneous tile placed. Assuming everyone in the game is competent (at or over the suggested age for Carcassonne) and that everyone in the game is actually conscious of the game at hand and not watching a movie, drinking too much or other such things, you should never ever let a ill-placed tile slip by.

I guess not, but what has happened a few times (with adult players) is that you - a couple of rounds after someone placed a farmer - realise that the field was connected to another with a farmer on. Not much to do then, if the mistake was made many rounds ago, than to pick up the farmer. (I must say that this happened once or twice when the game was pretty new to the players).
